The easiest way to complete the Daily Health Screening is by visiting the website https://healthscreening.schools.nyc/.
Families should complete the survey prior to arrival and take a screenshot of the completed screen (including the students/staff’s name and date) and show it to us when they get to the door.
To add the APP to your phone, visit the website https://healthscreening.schools.nyc
At the bottom it states “Add this tool to your phone.” When you click this it will ask you to select “Android” or “iPhone.”
